Personnel minimums need to participate:

10 rostered players (need USAU memberships and signed waivers)
1 rostered coach member (coach membership, signed waiver, cleared USAU background check)
1 rostered chaperone (cleared USAU background check, signed waiver, 21+ years of age)

Mixed States will be held at the Legacy Events Center in Farmington. We are hosting 16 high school teams. All teams must be registered for the event by Oct 21. With sunset at 5:15pm, plan on play starting as early as 12:30pm on Friday. This may change once registration closes and the number of teams are set. Team fees: $150 due by Oct 21 Payment Methods: - PREFERRED - PayPal UUDA through "friends and family" option to pay@utahultimate.org - Checks made out to "UUDA". Health, Safety and Liability Requirements - Please make sure to understand, follow, and communicate/share these requirements with all participants.

Youth Participation Requirements - These requirements must be followed for any event that involves a participant under the age of 18. This includes the requirement for a rostered chaperone.

Rostering Requirements - All event participants must be rostered through the USA Ultimate rostering system in order to participate. These instructions explain how to register teams and complete event rosters.
