Rounds start at 9, 10:30, 12, and 1:30. Captains / Coaches Meeting at 8:40.

Joel P Jensen Middle School

8105 S 3200 W, West Jordan - Directions

We will be using Southwest, South Central, Southeast, and East Central fields. The North fields will not be set up, so you will probably want to park on the south side of the school.

Bathrooms are available at Macey's Grocery Store, 1.5 blocks north on 3200 W.

We are excited to welcome your team to the Ultimate CHills Middle School Open Fall 2016 Tournament!

Requirements

Health, Safety and Liability Requirements - Please make sure to understand, follow, and communicate/share these requirements with all participants. The team chaperone should have a signed copy of a medical authorization form for each participant.

Youth Participation Requirements - These requirements must be followed for any event that involves a participant under the age of 18. This includes the requirement for a rostered, background-checked chaperone.

Rostering Requirements - All event participants must have a current waiver & membership and be rostered through the USA Ultimate rostering system in order to participate. Participants must be middle-school age (grades 5-8).
